Feedback is important for our practice. If you have a compliment, complaint or improvement suggestion, please complete the Contact Form which will be automatically emailed to the Practice Manager, and will be treated confidentially.
Your feedback enables us to pass on compliments, address complaints and use improvement suggestions. Thank you for taking time to provide your feedback. We will endeavour to respond to your feedback within 2 working days.
Please phone the centre if you require information regarding appointments, test results or doctor communication. To protect your personal information, we are unable to respond to emails received seeking medical advice, scripts, referrals or appointments.
Important Information for healthcare providers
We prefer to receive reports electronically rather than via email. Please communicate your reports through Healthlink. If you wish to communicate via any other platform, please contact us on 6924 3022 in advance to discuss. Please do not send patient health information via the contact form - this will not be responded to.
